Other possibilities could be intentional malicious actions by authorized people (insider threat), or a mistake by authorized people (human error, incompetence, negligence, …)

Though thinking about it some more: maybe the vagueness is actually more correct if it’s unknown if the interference was deliberate. I suppose it’s a small mercy that the headline wasn’t “unauthorised interference on Russian railway, you’ll never guess how many cars detailed”.
